504 Plan Builder

Build a school accommodation plan that knows what year it is — CGM, automated insulin delivery, phones as medical devices — with the source behind every ask and the words to use when someone says no.

📱 Works on phone, tablet, and desktop. Build the whole plan by tapping — then print it, or open it on a bigger screen when you want to edit the wording.

Step 1

Tell it about your kid

Tap through these. Every choice changes which accommodations show up and how the plan reads. Nothing here is required.

Grade band

Drives the self-management section and adds age-specific items.

Pronouns

So the plan reads like a document instead of a form.

Insulin delivery

Phone-controlled and automated systems unlock protections most template plans have never heard of.

Glucose monitoring

Emergency glucagon

How much can they do alone?

The ADA's guidance is explicit that this tracks development, not age. Pick what's true today.

Does the school have a nurse?

Be honest here. It changes the staffing language considerably.

Anything else true?

Tap all that apply. Each one adds accommodations built for that situation.

Names for the document

The only typing in this whole tool, and all of it is optional.

Step 2

What your state actually allows

This is the piece almost every family skips, and it decides how much of your plan is even possible. States differ enormously — some let trained non-nurse staff give both insulin and glucagon, some allow glucagon only, some say nothing at all. Tap your state, open the link, answer three questions.

Pick your state

Opens the American Diabetes Association's page for that state's laws.

Step 3

Choose your accommodations

Standard is on by default — language most districts accept without much argument. Ask for it takes a conversation. Stretch is a real ask that some districts grant and many won't. Tap any grey chip to see the source behind that item.

Step 4

Your plan

Click into the document and edit anything. Yellow marks are blanks that need a real number for your school — how many trained staff, how many minutes, how many days.

Read-only · tap Edit to change the wording
Step 5

The substitute teacher sheet

Your 504 is twelve pages. A substitute will read one. This is the one — built from your choices above, meant to live at the front of every sub folder and in the main office.

One page · print several
Step 6

What they'll say, and what to say back

None of this is adversarial. Most people across the table want to help and are working from a picture of diabetes care that's fifteen years out of date. Your job is to be the calmest, best-prepared person in the room. Tap any line to open the response.

Before you walk in

Sentences that should make you sit up

Said kindly, usually by someone who means well. Each one describes something federal guidance says a school can't do.

Before the meeting

Get your paper together

Walking in with these turns a negotiation into a review. Tap as you go.

Step 7

If they say no

Go in order. Skipping rungs costs you credibility, and the deadlines at the bottom are real and short.

Receipts

Everything this was built from

Nothing in this tool is invented. Every accommodation traces to one of these, and the grey chips throughout link straight to the relevant one.

🔒
Everything here stays on your device. This tool runs entirely in your browser. Nothing you enter — your child's name, school, devices, state, or anything else — is transmitted, stored on a server, or seen by anyone. "Save my work" writes to your own browser's local storage and nowhere else.
This tool is for educational purposes only. It is not medical advice, not legal advice, and not a substitute for your child's care team, your district's process, or an attorney or advocate. Nothing here should be used to make a treatment decision. Clinical details — doses, targets, ratios, thresholds, and timing — belong in a Diabetes Medical Management Plan signed by your child's prescriber, not in a 504 plan.
Laws on 504 eligibility, glucagon and insulin administration by non-nurse staff, and student self-management vary by state and change over time. Confirm what applies where you live using the state links above.
